No one from your organization will see your responses. Any information you share will be analyzed by us and reported in aggregate form. This means that all the results will be averaged across the larger group and no one individual will be identified with any single reply. To guarantee your anonymity, do not identify yourself by name or position in any of the comments you might choose to make on a survey. |

You will be asked to complete one or more of these surveys depending on the type of program you are participating in. Each survey contains 50 questions.

You may be asked to answer questions related to facilitation, leadership, teamwork, and/or trust. You may find these questions personal and we ask that you answer them honestly. If you do NOT wish to answer a particular question, please leave it blank and move on to the next one. Unless a question asks about a particular person, you may assume it is about you. Unless a specific group or team is identified, you may presume they are the group or team that you belong to. Unless specified otherwise, you are free to interpret group or team memberships as you like.  After finishing this survey, you will receive a personal profile based on your answers. You may print off a copy of this if you wish (your personal profile will not contain any identifying features). Once you have seen and/or printed a copy of your profile you are done and may simply close the window associated with this procedure.
